We are proud to provide Spravato® (esketamine), an FDA-approved nasal spray specifically designed for adults with Treatment-Resistant Depression (TRD).
 

What is Spravato?

Spravato is a prescription nasal spray used in conjunction with an oral antidepressant to treat adults with Treatment-Resistant Depression (TRD). It is typically recommended for individuals who have tried at least two or more antidepressant medications without significant improvement in their symptoms.

How is it Different from Traditional Antidepressants?
 

Most traditional antidepressants (like SSRIs or SNRIs) work by increasing levels of neurotransmitters like serotonin, norepinephrine, or dopamine. Spravato works differently:
 

  • Targets the Glutamate System: It acts on the N-methyl-D-aspartate (NMDA) receptor to increase the production of glutamate, the brain's most abundant chemical messenger.
     

  • Promotes Neuroplasticity: By modulating glutamate, Spravato helps "re-wire" and restore neural connections in the brain that may have been damaged by long-term depression.
     

  • Rapid Relief: While traditional medications can take weeks or months to work, many patients experience a reduction in symptoms within the first 24 hours to 4 weeks of treatment.



The Treatment Process at Psych Boston
 

Because Spravato can cause temporary sedation or a "disconnected" feeling (dissociation), it must be administered under medical supervision in our certified office.
 

  1. Self-Administration: You will self-administer the nasal spray under the guidance of our healthcare team.
     

  2. Observation Period: You will relax in a comfortable, quiet space in our office for at least two hours while our staff monitors your blood pressure and ensures you are reacting well to the medication.
     

  3. Safety First: Because of the temporary effects on alertness, you cannot drive yourself home. You must arrange for a friend, family member, or car service to pick you up. You should not drive or operate machinery until the following day after a restful sleep.



Treatment Schedule
 

Spravato is administered in phases to ensure the best possible long-term outcome:
 

  • Induction Phase (Weeks 1–4): Treatment twice a week.
     

  • Maintenance Phase (Weeks 5–8): Treatment once a week.
     

  • Ongoing Phase (Week 9+): Treatment every week or every two weeks, depending on your clinical response and our provider's recommendation.
